Transportation is provided to Virginia Premier Medicare Advantage Elite Plan members who require transportation assistance to approved healthcare services. Verida arranges transportation for all Virginia Premier Medicare Advantage Elite Plan members residing in counties and cities located throughout Virginia.
Call 855-880-3480 to arrange non-emergency medical transportation services to covered medical services at least three (3) business days in advance of the appointment date. You can schedule up to 30 business days in advance.

Making Transportation Arrangements
When scheduling transportation, please have the following information ready when you call:
- Your full name, phone number, address, date of birth, and county of residence
- Your Subscriber ID as it appears on your Member ID or your name & date of birth
- Specify any special needs (oxygen, escort) when you schedule the appointment
- Your emergency contact’s name and phone number
- Destination information – address, phone number, doctor’s name, type of appointment, and name of the medical facility
- Your mobility status (walking, wheelchair, or stretcher)
- Due to COVID-19, we recommend following CDC guidelines to wear a mask during transportation

NEMT is a service that is provided to Virginia Premier Medicare Advantage Elite Plan members who require transportation assistance to Medicare-billable healthcare services and have no other means of transportation.
How do I apply for Non-Emergency Medical Transportation?
Contact the Medicare office in your region to apply for Medicare or to find out more about Non-Emergency Medical Transportation.
How is transportation determined?
Verida arranges transportation for all Virginia Premier Medicare Advantage Elite Plan members residing in counties or cities located within the six regions of Virginia. Call the Verida at 800-880-3480 (TTY: 711) to schedule a ride.
Does Verida provide private transportation to appointments?
Since this is a shared-ride service, it is possible that you will ride with other passengers picked up at or near your pick-up location. Verida offers public transportation vouchers and fuel/gas reimbursement as well.
Are there limits to how far I can travel to and from a medical appointment?
There is no limit as long as it is within the state and it can also be out of state, as long as it is within 30 miles of the Virginia state lines
Am I allowed to bring shopping bags on the vehicle?
You are only able to carry one bag onto the vehicle; bag must fit in your lap…no exceptions.
Am I allowed to take a friend or family member to appointments?
One adult escort or attendant may accompany you if he or she has the same origin and destination as you and requires assistance for some medical reason. Escorts MUST be requested when scheduling the appointment.
When scheduling transportation, how much advanced notice is required?
To schedule transportation, you should book at least three (3) business days in advance of the appointment date. You may book up to 30 business days in advance.

Transportation is provided to and from medical appointments only; some of which may be considered covered or non-covered as determined by Medicare.
